MEDICAL TRANSCRIPTION
Transcribing medical dictations (digital voice) into text is called medical transcription (usually in a word document). The voices in the voice files are typically those of healthcare workers, primarily doctors or their assistants taking notes for them. They narrate notes on patient interactions, which are then provided in a digital format and typed by a trained medical transcriptionist after being listened to (transcribes).
Two goals are the main foci of medical transcribing. The transcript acts as a medical record that the doctor must present to the insurance provider in order for his claim to be authorised. Second, each document is a legal document and can be used as evidence for or against the doctor in the event that a medical negligence claim is made.
The spectrum of documents covered by medical transcribing includes:
• Notes on progress
• Management and evaluation notes
• Recommendation letters
• Transactional reports
• Summaries of discharge
• Medical history and tests
• Notes for consultation
• Laboratory and image reports
• Reports of diagnostic tests
• Autopsy findings
• Reports on radiology
• Insurance paperwork
• Mental health records
• Further notes
• Reports of adverse events
• Records of clinical trials.
• Progression updates
• Medical necessity letters
