                An individual who performs medical transcription (MT) compiles relevant, current, and private patient data into a written text document. This text can either be preserved solely in electronic form or added to the patient's file and kept in both electronic and printed form. 

Medical transcription can be carried out by MTs who work for hospitals as in-house or remote employees, by MTs who work as independent contractors or remote employees for an outsourced service that handles the task offsite for a hospital, clinic, physician group, or other healthcare provider, or by MTs who work onsite and remotely for the service providers (doctors or their group practices).


Hospitals usually select electronic storage of medical information since there are so many patients there and so much documentation that goes with them.
